How do I request maternity records from OLOL?

Hi Does anyone know how I do this? Is there an online form for OLOL or a postal address for maternity records for my dds birth? Thanks

you send in a letter with some form of ID( photo copy of your passport) with your name address and date of birth to the freedom of information section olol drogheda, requestin your maternity records and the date your little on was born. hope thats a help. i had mine in a about 2 weeks with no problem
