Maternity Benefit

Hi Girls form my ticker you will see that im nearly 19 wks pregnant! Just need advice as regards what forms do I need to complete for my maternity leave entitlements and when does it need to be completed? As always thanking you all in advance for your help! Hugo087

i'm just filling out my form at the moment. you have to be at least 24 weeks pregnant (i think!) for your doctor to fill out that part. so you don't need to do anything for a few weeks anyway! here's some details http://www.welfare.ie/EN/Publications/S ... apply.aspx
